About 1.7 people die every second on this planet. You can watch it happen in real time on our live death counter. On an ordinary day, roughly 150,000 people die — from heart disease, cancer, accidents, old age. The machinery of death is steady and predictable.
Pandemics are what happen when that machinery accelerates. When a new pathogen finds its way into a population with no immunity, the daily death count does not increase by a few percent. It multiplies. Entire cities empty. Entire civilisations collapse. And then, eventually, it ends — and the survivors rebuild on top of the dead.

The range in estimates — particularly for older pandemics — reflects the reality of counting deaths before modern record-keeping. The Black Death's toll is estimated between 75 and 200 million because 14th-century Europe did not have death registries. What we do have is archaeological evidence, parish records, tax documents showing depopulated villages, and contemporary accounts that are remarkably consistent in their horror.
1. The Black Death (1346–1353)
Nothing else comes close. The Black Death — caused by the bacterium Yersinia pestis, carried by fleas on rats — killed between a quarter and half of Europe's entire population in seven years. Some regions lost even more. Florence lost 60% of its citizens. Parts of England lost 70%.
The pandemic originated in Central Asia and travelled westward along the Silk Road, reaching the Crimean port of Caffa in 1346. From there, it spread by ship to Sicily, then Italy, then across the continent. Within five years, it had reached Scandinavia, the British Isles, and North Africa.
The scale is almost incomprehensible. Europe's population did not recover to pre-plague levels for 200 years. The labour shortage it created ended feudalism, doubled wages for surviving peasants, and fundamentally restructured European society. The world before 1346 and the world after were not the same world.
2. The Spanish Flu (1918–1920)
The deadliest pandemic of the modern era killed more people than World War I — the war it overlapped with. Unlike most influenza strains, which kill the very old and very young, the Spanish Flu was unusually lethal to healthy adults aged 20 to 40. It killed them through a catastrophic immune overreaction called a cytokine storm — the stronger the immune system, the more violent the response.

The second wave was the killer. In October 1918 alone, 195,000 Americans died. Philadelphia held a parade on September 28th; within six weeks, 12,000 Philadelphians were dead. The virus spread through the trenches of WWI, through troop ships, through the cramped conditions of wartime — conditions that selected for more virulent strains.
The name "Spanish Flu" is a misnomer. Spain was not the origin — it was simply the first country to report it openly, because Spain was neutral in WWI and did not censor its press. The virus likely originated in Kansas, USA.
3. The Plague of Justinian (541–549)
The first recorded pandemic of bubonic plague struck the Byzantine Empire at the height of its power. It arrived in Constantinople — then the largest city in the world — in 541 AD, probably from Egypt via grain ships carrying infected rats.
At its peak, the Plague of Justinian killed an estimated 5,000 people per day in Constantinople alone. Emperor Justinian I contracted the disease and survived, but his empire did not recover. The pandemic effectively ended his campaign to reunite the Roman Empire and marked the beginning of the Byzantine decline.
The same bacterium — Yersinia pestis — would return 800 years later as the Black Death, and again in the 1850s as the Third Plague Pandemic. A single species of bacterium, carried by a single species of flea, has killed more humans than any pathogen in history.
4. HIV/AIDS (1981–present)
Unlike the explosive pandemics above, HIV/AIDS is a slow burn — and it is still burning. Since the virus was first identified in 1981, it has killed over 42 million people. Another 39 million are currently living with HIV.
The geography of the pandemic is brutally unequal. Sub-Saharan Africa, home to 14% of the world's population, accounts for 67% of all HIV infections. In some southern African countries, life expectancy dropped by 20 years during the worst decades of the crisis — a demographic collapse visible in our life expectancy data.
The development of antiretroviral therapy in the mid-1990s transformed HIV from a death sentence into a manageable chronic condition — for those who can access treatment. In wealthy countries, a person diagnosed with HIV today can expect a near-normal lifespan. In parts of Africa, treatment access remains inconsistent.
5. The Third Plague Pandemic (1855–1960)
The longest pandemic in modern history began in Yunnan province, China, in 1855 and was not declared over until 1960. It killed an estimated 12 to 15 million people, the vast majority in India and China.
This pandemic is notable for one breakthrough: in 1894, during an outbreak in Hong Kong, Alexandre Yersin identified the bacterium responsible — Yersinia pestis, named after him. For the first time, humanity understood what was actually killing it. The discovery led to rat-control programmes, quarantine protocols, and eventually antibiotics — tools that ended plague as an existential threat.
6. Smallpox in the Americas (1520–1600s)
When Europeans arrived in the Americas, they brought diseases that Indigenous populations had never encountered. Smallpox, measles, typhus, and influenza swept through civilisations that had no immunity whatsoever.
The result was the greatest demographic catastrophe in recorded history. The pre-contact population of the Americas — estimated at 50 to 60 million — was reduced by 90% within a century. The Aztec Empire, which had withstood military conquest, was destroyed by smallpox. Tenochtitlán's population dropped from 300,000 to fewer than 30,000 in a single generation.
This was not biological warfare in most cases — it was an unintentional consequence of contact between populations separated by 12,000 years of independent evolution. The pathogens did not care about intention. They simply found a population with no defences and did what pathogens do.
7. COVID-19 (2020–2023)
The first pandemic of the information age killed an estimated 7 million people by official counts — though excess mortality studies suggest the true toll was closer to 15 to 25 million. The gap between official and estimated figures reflects the reality that many countries lacked the testing infrastructure to count every death.

COVID-19's relatively lower death rate compared to historical pandemics is partly a function of modern medicine — ventilators, antivirals, and vaccines developed at unprecedented speed — and partly because the virus, while highly transmissible, was less lethal than many historical pathogens.
What pandemics have in common
Despite spanning 2,000 years, the deadliest pandemics share a remarkably consistent pattern.
Zoonotic origins. Every pandemic on this list originated in animals before jumping to humans. Plague came from rodents. Influenza from birds and pigs. HIV from chimpanzees. COVID-19 almost certainly from bats. The human-animal interface is where pandemics are born.
Trade routes as transmission routes. The Black Death followed the Silk Road. The Third Plague followed shipping lanes. Spanish Flu followed troop movements. COVID-19 followed airline routes. Pandemics are the dark side of connectivity — they travel wherever humans travel, at the speed humans travel.
Unequal impact. Every pandemic kills the poor disproportionately. Crowded housing, poor sanitation, limited healthcare access, and malnutrition all amplify death rates. This was true in 1346 and it was true in 2020.
Societal transformation. The Black Death ended feudalism. Smallpox in the Americas enabled European colonisation. The Spanish Flu accelerated public health systems. COVID-19 reshaped remote work, global supply chains, and vaccine technology. Pandemics do not just kill — they restructure the civilisations that survive them.
Why the next pandemic is when, not if
Epidemiologists are not divided on whether another pandemic will occur. They are divided on when and what.
The conditions that produce pandemics — dense human populations, expanding contact with wildlife, global air travel, intensive animal farming — are all more prevalent today than at any point in history. The WHO's Disease Outbreak News database currently tracks over 50 active pathogen outbreaks at any given time.
What has changed is our capacity to respond. mRNA vaccine platforms can produce a candidate vaccine within 48 hours of a pathogen being sequenced. Genomic surveillance networks can detect novel pathogens within days. Antivirals and monoclonal antibodies provide treatment options that did not exist a generation ago.
The question is whether these tools will be deployed fast enough, equitably enough, and at sufficient scale. The record on that, as COVID-19 demonstrated, is mixed.
The total toll
If you add up every pandemic in recorded history — including smaller outbreaks not listed above — the cumulative death toll is somewhere in the range of 300 to 500 million people. To put that in perspective, an estimated 109 billion humans have ever lived. Pandemics have killed roughly 0.3–0.5% of all humans who ever existed.
That percentage sounds small until you remember that pandemics compress centuries of death into years, that they strike without regard to merit or effort, and that every number in every table above was a person who woke up one morning not knowing that a microscopic organism had already decided their fate.
