Heart attacks vs strokes

People use "heart disease" as a catch-all term, but the data breaks down into distinct conditions, each with its own death toll.

Heart attacks alone kill 8.9 million people per year — roughly 24,384 per day. Strokes kill another 6.6 million. Together, these two conditions account for 83% of all cardiovascular deaths.

The distinction matters because they have slightly different risk profiles. Heart attacks are more common in men and tend to strike a few years earlier. Strokes are more equal between genders and more closely linked to high blood pressure than to cholesterol.

The numbers in the "population affected" column are staggering. 1.3 billion people have high blood pressure. 2.6 billion have elevated cholesterol. 1.4 billion are physically inactive. These are not rare conditions — they are the norm in most developed and middle-income countries.

What the data also shows is that these risk factors are multiplicative, not additive. If you have high blood pressure AND high cholesterol AND you smoke, your risk is not 2.0 + 1.8 + 2.2 = 6x higher. It is roughly 2.0 × 1.8 × 2.2 = 7.9x higher. The factors compound each other.

This is why someone can have a heart attack at 45 with "slightly" elevated blood pressure, "slightly" elevated cholesterol, and a "moderate" smoking habit. Each factor alone is manageable. Together, they are lethal.

Japan's cardiovascular death rate is roughly six times lower than Uzbekistan's. The Japanese advantage comes from diet (high fish consumption, low saturated fat), lower obesity rates, universal healthcare, and cultural factors around physical activity.

Russia and Eastern Europe stand out painfully. The combination of high smoking rates, heavy alcohol consumption, poor diet, and limited preventive healthcare creates a cardiovascular crisis that has been shaving years off life expectancy for decades.

Heart disease is not just an old person's problem. It starts much earlier than most people think.

Eleven percent of cardiovascular deaths happen before age 55. These are not edge cases — that is over 2 million people per year dying of heart disease in what should be the prime of their life. And for every person who dies, several more survive a heart attack or stroke with permanent damage.

The disease process — atherosclerosis, the gradual buildup of plaque in your arteries — begins in your twenties. Autopsies of young soldiers killed in the Korean and Vietnam wars found that the majority already had visible arterial plaque. By the time a heart attack happens, the underlying disease has typically been developing for 20 to 30 years.

The warning signs most people miss

Heart attacks are not always dramatic. In fact, the Hollywood version — clutching your chest and collapsing — represents the minority of cases. Many heart attacks are silent or present with symptoms people dismiss.

  • Shortness of breath during normal activities
  • Chest discomfort that feels like pressure, not sharp pain — often mistaken for indigestion
  • Pain radiating to the arm, jaw, or back — particularly the left arm
  • Cold sweats and nausea without obvious cause
  • Dizziness or lightheadedness

Women are particularly likely to experience atypical symptoms — jaw pain, nausea, fatigue, and back pain rather than classic chest pain. This is one reason women are more likely to delay seeking treatment and more likely to die from their first heart attack than men.

What actually works

The good news — and there is genuine good news here — is that cardiovascular disease is largely preventable. Countries that have made a serious effort have achieved dramatic reductions.

The striking thing about this table is that lifestyle changes — exercise, diet, quitting smoking — are as effective as medication. And they are cumulative. A person who exercises regularly, eats well, does not smoke, and maintains a healthy weight has a cardiovascular risk roughly 80% lower than someone who does none of these things.

Since the 1960s, countries like the US, UK, Finland, and Australia have cut their cardiovascular death rates by more than 50%. Finland's North Karelia project — a community-wide intervention targeting diet, smoking, and blood pressure — reduced heart disease mortality by 80% over four decades. It remains one of the most successful public health programmes ever conducted.

Cardiovascular disease kills more people than cancer, road accidents, malaria, HIV, suicide, and diabetes combined. It is the world's leading cause of death, the world's leading cause of disability, and the world's most expensive disease to treat.
